NOTICE OF APPOINTMENT OF THE PERSON IN CHARGE OF THE PROTECTION OF PERSONAL INFORMATION

In accordance with Québec’s Law 25 on the protection of personal information in the private sector, Physio Élite appoints:

• Audrey-Amélie Perron as the person in charge of the protection of personal information for the Physio Élite inc. clinic in Laval
• Laurie-Anne Bonneau as the person in charge of the protection of personal information for the Physio Élite St-Jérôme clinic
• Lilian Halme as the person in charge of the protection of personal information for the Physio Élite Saint-Sauveur clinic

These persons will ensure that best practices in the management of personal information are always in effect within the company.

It is also in their capacity as persons in charge of the protection of personal information that they will receive and process all requests for access, rectification or de-indexing of personal information from a client or an employee of our company.

You may contact the person in charge of the protection of personal information at any of our clinics at any time.
